Best Practices for Pressure Wound Staging in Home Health: Explained By a Wound Care Specialist

Are your clinicians staging pressure injuries correctly—or setting your agency up for preventable complications and denials?

This podcast by THCC breaks down pressure-injury staging in plain English for home health, with guidance from Mariam Treystman and Dr. Robert Goldberg. It explains the most common staging misconceptions, why they’re risky for patient outcomes and compliance, and the simple cues that help teams stage accurately and document cleanly.

📌 You’ll learn:

• Why correct staging affects interventions, infection risk, hospitalizations, and survey outcomes

• The key rule: tissue layers ≠ staging (and why that mix-up drives under-staging)

• Stage 1–4 quick visual definitions (intact redness → partial-thickness → fat → muscle/fascia/bone)

• The anchor that prevents errors: “Muscle = Stage 4”

• Why coding/billing structure can mislead teams—and how to avoid billing pitfalls

What makes us different?

Wound care led by a cardiothoracic surgeon—built around the patient, not the spreadsheet.

Dr. Robert Goldberg is a formally trained cardiothoracic surgeon who chose to devote his practice to advanced wound care. His approach is both clinical and human: he treats the wound with precision, while educating home health teams, nurses, patients, and families so everyone understands the plan and can support healing between visits. Dr. Goldberg views a wound as a symptom—not the whole diagnosis. The goal isn’t just closure; it’s identifying and addressing the “why” behind the wound (circulation, pressure, infection risk, nutrition, mobility, and underlying disease), so healing lasts.

Yes! Dr. Goldberg is fluent in English & Russian.

Born and raised in Russia, Dr. Goldberg completed 5 years of medical school at the Russian State Medical University in Moscow and was building his career as a Cardiac Surgeon. However, in 2008 he immigrated to the United States and had to redo his entire education from scratch, earning his medical degree from Cooper Medical School of Rowan University in 2016. He was then trained in adult and congenital Cardiothoracic Surgery for four years at LAC + USC Medical Center.

Attributing his success to perseverance and having always been inspired by the medical field, Dr. Goldberg served as a congenital cardiothoracic surgery research fellow at Children’s Hospital of Los Angeles until December 2019. In 2020, he left Cardiothoracic Surgery to pursue a different career path. Thereafter, from 2020 until 2022, he worked as a traveling physician for Signify Health. He started his wound care practice later in 2022.
